| 1-49 | $2.00 per Card |
| 50-99 | $1.25 per Card |
| 100+ | $1.00 per Card |
Visa, MasterCard, American Express and Discover accepted.
Cash and personal checks not accepted. Corporate checks are accepted for payment if accompanied by a valid state driver's license.
